Health Screening Forms and Player Medical Form

The Health Screening Forms are now available from the website’s top menu.  Please go to the “Hockey Parents” tab.  There you should see a “Return to Play-Covid-19” tab, which you will find our BMHA Return to Action Plan approved by the OMHA (which I strongly encourage parents to read), BMHA Declaration, and Guidelines/Screening/Waiver/Video of the Municipality of Central Elgin.  

There will be two Health Screening Forms that will be required for each person (every player, coach, referee, BMHA board member, spectator, etc.) entering the Arena.  That would be the BMHA Declaration and Central Elgin’s Screening/Waiver.  For simplicity, I have attached those two documents below:  



Please print both forms off and complete them on the day of your scheduled ice time….NOT in advance!  You will be required to bring these forms to the Arena for the first scheduled ice time and hand them to a Board Member and/or Covid-19 Team Manager who will be sitting at a table when you enter the Arena.  Any scheduled ice time after the first one, you will only be required to bring the Central Elgin Screening and Waiver form for every person entering the Arena.

MOST importantly, if you or anyone in your household is showing symptoms or awaiting a Covid-19 test result, please DO NOT COME TO THE ARENA and advise your team coach or manager ASAP. 

In addition to the above, for the first scheduled ice time ONLY, you will also be required to please bring the Player Medical Form.  I have attached that document below:  


This will also get handed to a Board Member and/or Covid-19 Manager who will be sitting at a table when you enter the Arena.  For future reference, the Player Medical Form can also be found under the “Hockey Parents” tab.
