The Belmont Minor Hockey Association (“BMHA”) requires the disclosure of exposure or illness in order to safeguard the health and safety of all hockey participants and limit the further spread of Covid-19. As such, we are asking that a Declaration of Compliance be completed by each participant prior to participating in each on-ice or off-ice activity. This Declaration of Compliance will be kept safely, and personal information will not be disclosed unless as required by law or with your consent.

Municipality of Central Elgin
The Municipality of Central Elgin has put in place guidelines to protect the safety of all staff and the public. You can find their document at the link below. As part of these guidelines, the Covid-19 Screening/Waiver must be completed by each individual prior to participation in each on-ice or off-ice activity.
All parents/guardians/children/participants and coaches/bench staff are required to do a self-assessment on the day of their scheduled hockey ice time and then complete the Covid-19 Screening Waiver. A Covid-19 representative will be present to check temperatures, make sure the mask policy is being adhered to, and to ensure that forms are being completed. One parent/guardian must be present during the check in process and if over the age of 18, the participant has the legal right to sign-off without a parent or guardian present.
