Police Checks (Belmont Minor Hockey)

Police checks - Anyone involved in coaching, training, etc. MUST have a valid police check in order to be on the bench or ice with the players. If you do not have a police check, you will not be permitted on the bench or the ice during practices or games.  BMHA is working hard to serve you and we are asking that all forms be returned no later than October 31, 2017.  Please contact an executive member in order to obtain a volunteer letter that you can take to the police/ opp station. Please do not wait until the last minute to have this done if you think you may want to help this year. There is no cost to you to have this done, simply some time to drop off a form and pick it up a few weeks later.  If you have a copy of one that you recently completed BMHA will also accept this.  These record checks will be considered valid for three years. 

If you have already picked up this package- then the form that is attached to your letter is no longer valid.  You can pick up a new form at the OPP office or at the following link:

The letter is still valid and will be required in order to have your police check done.
