
Proof of Vaccination for the 2021-2022 Hockey Season
In accordance with the OMHA Vaccination Policy, BMHA will require proof of vaccination status for all individuals, 12 years of age or older, to be able to participate in the 2021-2022 season. This will include players, coaching staff, officials, and associated volunteers.
On November 1, 2021, any affected person not fully vaccinated (i.e.: including the 14-day period after receiving their completed dose) or to which who has not been granted an accommodation, will no longer be eligible to participate in any BMHA sanctioned activities until such time as they have presented satisfactory evidence that they have been fully vaccinated or been granted an accommodation.
All affected persons need to provide proof to BMHA, who will then document vaccination status accordingly. BMHA will not keep any medical records or vaccination receipts on file, but rather simply confirm that proof has been provided and record and maintain a list of all participants who have provided such proof.
Acceptable documents serving as evidence of COVID-19 vaccination include:
(i) A digital or physical Dose Administration Receipt;
(ii) Medical records signed by a licensed health care provider indicating vaccine name and date(s) of administration; or
(iii) Government verification/language (e.g. Government issued vaccine passport or card).
Proof of vaccination administration can be directly emailed to Sandy Richardson at [email protected]. On November 1st, for those who have not emailed Sandy will have to verify their proof of vaccination in person at the time they enter the arena.
